Infertility tests for male

The male role in pregnancy is to produce good amount of healthy sperms and to deliver them effectively into the vagina.

The tests to be done for male are to assess these functions.

  1. The first thing a doctor will do is taking a history about your medical illnesses and doing general physical exam of the body and testes.
  2. The next step and probably the most important is to do a semenfluid analysis.

The semen fluid analysis should be done as follows:

  • Abstinence for 3 days at least, preferably 5 days from sexual intercourse before giving the sample.
  • The sample should be given fully and nothing should be spelled.
  • The sample is better to be given manually using masturbation and not using intercourse.

It should be noticed that the first part of semen contains most of the sperms.

 What are the normal sperm parameters ??

I will summarize the normal semen analysis in the following table according to world health organization 2010 which is the latest.

 

VOLUME >1.5 ML
pH 7.2-7.8
CONCENTRATION >15×106/ML
MOTILITY >42%
MORPHOLOGY >4% WITH NORMAL MORPHOLOGY
WBC < 1×106/ML

 

For motility it should be > 42% for fast progressive motility and slow progressive motility added together.

LIQUEFACTION TIME. Semen is a thick gel at the time of ejaculation and normally becomes liquid within 5-30 minutes after ejaculation. Liquefaction time is a measure of the time it takes for the semen to liquefy.
> Normal: 5-30 minute after collection more than that it will be called viscous which may impair penetration of the cervix.

Agglutination: should be negative

3-Hormonal testing may be done for testosterone, prolactin and FSH as a start.

 

4- transrectal and scrotal ultrasound to assess for obstruction and varicocele.

 

5- Post ejaculatory urine analysis to assess for retrograde ejaculation where the sperms go retrogradely into the bladder.

The semen liquefies within 5–25 min at room temperature or in an incubator, some specimens may take longer and make the semen difficult to analyze.
